In this Welcome Pack, you will find information about the great team we have at the Practice.
We have a Dental Charter so that patients know what they can expect from us and what we expect from them. Treatment that we provide is summarised along with the various payment options we offer including Denplan and Practice Plan.
We have also attached a medical history form, a smile check questionnaire and a dental history questionnaire for new patients. Should you wish to join the Practice, we would be grateful if you would download these forms and bring them along with you to your appointment.
